How does the transition into a care home work?

We will create a personalised care plan and offer opportunities to visit the home, meet the team, and get familiar with the environment.

On move-in day, our staff will be ready to welcome you, help settle you in, and provide gentle support. We stay closely involved to ensure you feel safe, comfortable, and at home.


Whats the process?

Step 1: Initial Enquiry & Home Visit

The journey often begins with a simple conversation. Whether you’re exploring options for yourself or a loved one, we welcome enquiries by phone, email, or in person. We will invite you to tour the home, meet the team, and get a feel for the environment. This is a chance to ask questions, see how we work, and decide whether our approach aligns with your needs and values.

Step 2: Care Needs Assessment

If you’re considering a move, the next step is a care needs assessment. This can be arranged through your local authority or privately, and helps determine the level and type of care required. It looks at physical health, emotional wellbeing, daily living needs, and any medical conditions. We will use this information to ensure we can meet your needs safely and respectfully.

Step 3: Financial Assessment & Funding Guidance

Alongside the care assessment, a financial assessment may be carried out to determine eligibility for funding support. We will talk you through the process, explain what’s included in our fees, and help you understand your options—whether you’re self-funding, applying for local authority support, or exploring NHS Continuing Healthcare.

Step 4: Personalised Care Planning

Once care needs and funding are confirmed, we will work with you and your family to create a personalised care plan. This includes preferences around daily routines, meals, activities, and any specific support needs. We believe care should reflect the individual, not just the condition.

Step 5: Preparing for the Move

We will help you prepare for the transition—whether that means setting up your room with personal items, or simply offering reassurance. Families are encouraged to stay involved, and we will keep communication open throughout.

Step 6: Settling In

On move-in day, our team will be ready to welcome you with warmth and support. We offer a gentle settling-in period, regular check-ins, and opportunities to join activities at your own pace. It’s not just about care—it’s about feeling at home.
